Admission Criteria

 

The patient must be under the care of a physician who must give an order for hospice care.

 

The patient must have a life-limiting illness with an expectancy of six months or less to live, as determined by the hospice medical director and the attending physician.

 

The patient family/caregiver desire and willingness to participate in the plan-of-care and sign the consent form for care.

 

Care will be provided at either of the location patient's home, a family member's home, a skilled nursing facility, or other living arrangements.


Care for your loved ones will be provided without discrimination on the basis of race, religion, age, sexual orientation, disabilities, communicable diseases, or place of national origin.

 

Eligibility criteria will be reviewed on an on-going basis by the interdisciplinary team to assure appropriateness of hospice care.
